Preschool & Kindergarten ASQ Requirements

Hello, Prekindergarten and Kindergarten Parents/Guardians! 

All PreK 3, PreK 4, and Kindergarten parents/guardians are required to complete the Ages and Stages Questionnaires (ASQ) to assess PreK and Kindergarten readiness. These questionnaires will also assist staff in getting to know your student.

You may use the buttons below to complete the questionnaires at home with your student prior to the first day of school on August 18, 2026. When completing the questionnaire, it may be helpful to have your student nearby to assess simple skills. Please follow the instructions below to complete BOTH the ASQ-SE2 and ASQ 3 from the appropriate link below. Once the first form is submitted, select "Click here to complete your ASQ:SE-2 screening now."

Directions:

Select the button for your child’s school. You will then be directed to:

1. Enter your child’s birthday in the following format (with dashes): YYYY-MM-DD (example: 2015-05-09)
2. Enter the number of weeks your child was premature. If they were not premature, enter “0.”
3. For the screening dates, choose “I am screening my child today.”
4. Click “Submit.”
5. Click “Enter your results.” (You do not need to print out the hard copy ASQ-SE2/ASQ-3 to proceed.)
6. Go through the questionnaire and answer to the best of your ability. If you aren’t sure, have your child try to complete the activity. 

The system will have you complete the ASQ-SE2 first. When you finish with ASQ-SE2, there will be a link to complete ASQ 3. Please complete both.
