Preschool ASQ Requirements
Hello, Preschool Parents/Guardians!
New for the 2025-2026 school year, all PK3 and PK4 parents/guardians are required to complete the Ages and Stages Questionnaires (ASQ) to assess preschool readiness. These questionnaires will also assist staff in getting to know your student.
You may use the link to complete the questionnaires at home with your student. When completing the questionnaire, it may be helpful to have your student nearby to access simple skills. Please follow the instructions below to complete BOTH the ASQ-SE2 and ASQ 3 from the appropriate link below. Once the first form is submitted, select "Click here to complete your ASQ:SE-2 screening now."
