Step Up to Kindergarten/ASQ
Hello, Kindergarten Parents/Guardians!
The State of Kansas requires all incoming kindergarten parents/guardians to complete the Ages and Stages Questionnaires (ASQ) to assess kindergarten readiness. These questionnaires will also assist staff in getting to know your student. We have two options for you to choose from!
Online -> You may use the link to complete the questionnaires at home with your student. When completing the questionnaire, it may be helpful to have your student nearby to access simple skills. Please follow the instructions below to complete BOTH the ASQ-SE2 and ASQ 3 from the appropriate link below. Once the first form is submitted, select "Click here to complete your ASQ:SE-2™ screening now."
OR
In Person -> Bring your student with you to the Step Up to Kindergarten events during onsite enrollment at your zoned elementary school. (Please allow approximately 30 minutes for completion.)
