Patton AVID Student Survey

To the Families of Patton AVID Students,

From December 15-19, 2025, our Patton AVID classes will administer a locally developed AVID student feedback survey as part of our AVID program implementation process. Student survey responses are anonymous, and no personally identifiable student data will be collected.

A parent/guardian must “opt-in” with your consent for your child(ren) to complete the survey. This “opt-in” for the “AVID Student Survey Parent Permission Form” is found at the link below.

Please complete a permission form for each of your children enrolled in an AVID course at Patton by December 12, 2025. Students with permission to take the survey will receive their survey from their AVID teacher by December 19, 2025. Students who participate in the survey will answer questions based on if they have had AVID instruction at a previous school or if they have only had AVID instruction while attending Patton. Both surveys are attached above for your reference.
